Booking an appointment

To book an appointment, please call 312.664.8376.

If you are unable to speak to someone, please leave a message on the appointment line and your call will be returned as soon as possible.

Holistic Health Practice schedules many clients a day. In order to ensure uninterrupted service for each client, appointments are required.


Cancellation Policy

Cancellations/rescheduling must be made 24 hours prior to scheduled appointments. Clients will be charged the full session rate for any cancellations made after this period.
A valid credit card and current address are kept on file for all new patients and those who book appointments for 2hours or longer.


Phone Sessions

Phone counseling sessions are offered by Kurt Hill and Holistic Health practitioners. (Please see rate sheet for details).
Payments for these sessions are made 24 hours prior by an advance credit card transaction. It is the responsibilty of the client to call the office at the designated appointment time.


Fees

The cost of each session is listed on the Session Rate sheet. Additional fees apply for first time clients and those who receive service before/after regular office hours.

We request first time clients to arrive at least 15 minutes prior to their scheduled appointment to complete necessary paperwork.

First time clients may request a fax copy of this paperwork in order to fill it out at their convenience and bring it with them at the time of arrival.

Sessions requiring special services will be charged a set rate determined on a case-by-case basis to be discussed with the client before services are administered.


Payment

We accept personal checks, cash, Mastercard and Visa credit cards. Checks may be made out to Holistic Health Practice for all practitioners except for Dr. Amie Shimmel.
